Vehicle Check-In/Check-Out Agreement

This Agreement ("Agreement") is entered into on [Date] by and between CU Midwest Designs, hereinafter referred to as "Company," and [Renter's / Subcontractors Name], hereinafter referred to as "Renter / subcontractor," collectively referred to as "Parties." complete areas below and submit contract. 1. Vehicle Care and Maintenance:

1.1 Subcontractor agrees to use the rented vehicle with utmost care and responsibility. Any damage to the vehicle, including but not limited to interior damage or exterior damage due to negligence, cuts or stains on seats, or paint damage to seats, shall be the sole responsibility of the Subcontractor.

1.2 Smoking of any substance is strictly prohibited inside the vehicle. Subcontractor shall not smoke or allow others to smoke in the vehicle. A cleaning fee of [$250.00] will be charged if smoking is detected. More charges may apply.

2. Fines:

2.1 In the event of any damage to the vehicle that is of the fault of the driver, Subcontractor agrees to pay for all repair costs and associated expenses.

2.2 Fines for common damages are as follows (but not limited to):

  • Interior Damage (per incident): [$650.00] or cost for professional fix. - This will be determined by CU Midwest Designs Estimator.

  • Cuts or Stains on Seats (per incident): [$650.00] or cost for professional fix. - This will be determined by CU Midwest Designs Estimator.

  • Paint Damage to interior or within exterior truck bed (per incident): [$650.00] or cost for professional fix. - This will be determined by CU Midwest Designs Estimator.

2.3 Any fines will be back-charged to subcontractor

3. Trash Removal:

3.1 Subcontractor is responsible for removing all trash and debris from the vehicle at the end of each workday.

3.2 Failure to remove trash will result in a fine of [$100.00] per incident.

4. Inspection and Check-In/Check-Out Procedure:

4.1 Subcontractor agrees to inspect the vehicle at the time of check-in and check-out, noting any existing damage.

4.2 Any damage discovered during the check-in process that was not previously documented will be the responsibility of the subcontractor.

5. Termination of Agreement:

5.1 The Company reserves the right to terminate this Agreement at any time for violation of any terms outlined herein.

6. Governing Law:

6.1 This Agreement shall be governed by and construed in accordance with the laws of [Illinois].

7. Entire Agreement:

7.1 This Agreement constitutes the entire agreement between the Parties and supersedes all prior or contemporaneous agreements, understandings, and representations, whether oral or written.

  8. Acceptance of Terms:

8.1 By filling out this form and submitting it, the Subcontractor acknowledges that they have read and understood the terms of this Agreement and agree to be bound by its provisions.

8.2 This Agreement shall come into effect upon submission of this form and shall remain in effect until the vehicle is returned to the Company in accordance with the terms outlined herein.

8.3 Subcontractors electronic submission of this form shall be considered equivalent to a physical signature and will be legally binding.
